Filling Out Chinese Visa Application Form – Instructions + Example

How to get a Chinese visa

To avoid your China visa being rejected, applicants should figure out the best answers to the questions in the application form. Here are some tips for filling out a Chinese Visa Application Form.

Current Occupations and Employer/School

Section 1.15 Current Occupations & Section 1.17 Employer/School: If you choose “Self-employed”, do not leave Section 1.17 blank. You need to complete this section with the name of your business and specify the nature of it. If you are unemployed at present, provide information on your previous employer or school if you are a student. If you are a housewife or retired, you can fill in the box in Section 1.17 with None (Housewife) or None (Retired).

Education

Section 1.16 Education: It does not matter what your answer is, unless you apply for a student visa. The best way is to check the highest level of education you have completed.

Major Purpose of Visit

Section 2.1 Major purpose of your visit: Select the most suitable item and make sure it matches with your application documents. If your travel is for multiple purposes, just choose one major purpose.

Intended Number of Entries & Duration of Stay

Section 2.2 Intended number of entries: Most applicants only need one entry valid for 3 months. Note that all Z, X1, S1, and Q1 visas are only issued as single entry. If you apply for an L, F, M, X2, S2, or Q2 visa, you may choose single-entry, double-entry, or multiple entries. Choose the one that best meets your needs. The final decision is left to the consular officer’s discretion.

Section 2.5 Longest intended stay in China among all entries: For L, M, and F visas, the duration of stay is normally 30-60 days. For S2 and Q2 visa, it ranges from 30 to 120 days. Z, X, Q1, and S1 visas may allow a longer stay duration. Fill in the box according to your actual need

Expected Date of Arrival in China

Section 2.4 Expected date of your first entry into China on this trip: Make sure that the date is not earlier than the date you are likely to receive the visa. If your flight itinerary will be submitted as the application document, ensure the date is consistent with it.

Travel Itinerary

Section 2.6 Itinerary in China: In this section, do not mention travel to Tibet and Xinjiang although you may go there. If you apply for a tourist visa, you need to provide addresses of hotels including provinces, cities, and streets. For this reason, you are advised to book refundable hotel rooms in advance. After you get the visa, it’s fine to change your itinerary and cancel the bookings

Background Check

In Part 3, you need to answer truthfully the questions regarding overstaying, China visa rejection history, criminal records, diseases, etc. Honesty is important in answering these questions; otherwise you may encounter problems in the application process

Only Typewritten Application Forms Are Accepted

Applicants need to fill out the application form on a computer first and then print it out. Double-sided printouts will not be accepted. Note that the signatures and dates in Part 4 and 5 must be handwritten.

Section 1

1.1 Please fill in your name as in your passport and separate your first name, last name and middle name.

1.2 Please select your sex.

1.3 Please fill in your Chinese name, if applicable.

1.4 Please fill in your current nationality as in your passport.

1.5 Please fill in your other Chinese name or foreign maiden, religious, professional, aliases names if applicable.

1.6 Please fill in your former nationality if applicable.

1.7 Please fill in the date of your birth as in your passport.

1.8 Please fill in the name of your birth place (City, Province/State and country).

1.9 Please select the type of your passport with which you are applying for the visa. If you use travel document other than a passport, please specify.

1.10 Please fill in the number of your passport with which you are applying for the visa.

1.11 Please fill in the date of issue of your passport.

1.12 Please fill the name of place (City, Province/State and country) of issue of your passport.

1.13 Please fill in the expiration date of your passport.

1.14 Please select your current occupation(s).

If your occupation(s) are not listed above, please specify the details.

Section 2

2.1 Please select the purposes of your visit(s) to China. If your purpose(s) to China are not listed above, please specify the details.

2.2 Please select the number of entry of your visit(s) to China with the visa. Visa normally comes into valid from the date of application.

2.3 The date of your possible entry into China.

2.4 Please fill in the number days among the entries you intend to stay in China. If the entries are more than 2, please fill in the number of your longest stay.

2.5 Please list the name of cities/counties, provinces you intend to visit in China in a time sequence.

2.6 Please select the intended date of pickup of the visa.

Section 3

3.1 If you have been refused for a Chinese visa in or out of China, please select YES. Otherwise, please select No.

3.2 If you have been refused to enter into or been deported from China, please select YES, please select No.

3.3 If you have any criminal record in any country including China, please select YES. Otherwise, please select No.

3.4 If you suffer from any one of the following diseases, please select YES. Otherwise, please select No.

3.5 If you have visited China before, please select YES. Otherwise, please select No.

3.6 If you select YES to any questions from 3.1 to 3.4, please specify the details.

Section 4

4.1 Please fill in the full name of your employer or school.

4.2 Please fill in your daytime phone number.

4.3 Please fill in the post address of your employer or school.

4.4 Please fill in your nighttime phone number.

4.5 Please fill in the post address of your home address.

4.6 Please fill in your email address.

4.7 Please fill in the name of the company in China which invite your visit, or the name of your reception unit in China, or the name of the School you intend to study in, or the name of your relatives or friends in China whom you intend to visit.

4.8 Please fill in the phone number of the company, school or person filled in item 4.7.

4.9 Please fill in the post address of the company, school or person filled in item 4.7.

4.10 Please fill in the email of the company, school or person filled in item 4.7.

Section 5

If you have more information relevant to your application to declare , please specify the details.

Section 6

6.1 Please fill in the name of the person filling out the form for the applicant.

6.2 Please fill in the relationship of the person filling out the form and the applicant.

6.3 Please fill in the post address and phone number of the person filling out the form.

6.4 The person filling out the form should sign.

Section 7

Applicant should read carefully and signed to agree and fill in the date.

ALL applicants MUST sign except the minor under 18 and whose form is prepared by his or her legal guardian.
